We are pleased to announce that the National Committee for Quality Assurance (NCQA) has recognized our organization as a Patient-Centered Medical Home.
What is a Patient-Centered Medical Home?
PCMH is a model of care in the primary care setting that emphasizes high quality healthcare including care coordination, improved communication, and patient/family participation. Research has proven that medical homes lead to higher quality care and improve patient experiences.
“The patient-centered medical home raises the bar in defining high-quality care by emphasizing access, health information technology and partnership between clinicians and patients” said NCQA President Margaret E. O’ Kane.
